A Smith fracture is a break of the radius just above the wrist where the broken end tilts forward toward the palm. It's sometimes called a reverse Colles' fracture and often needs surgical fixation because it tends to be unstable.

How it differs from a Colles' fracture

In a Colles' fracture, you fall onto an outstretched hand with the wrist bent back, and the fragment tilts backward. In a Smith fracture, you fall onto a flexed wrist, or take a direct blow to the back of the wrist, and the fragment tilts forward. They're less common than Colles' fractures but more often unstable, so plate fixation on the palm side is frequently used.

After surgery or casting

Early in recovery, finger, elbow and shoulder movement prevent stiffness from spreading. Once your surgeon allows it, wrist exercises begin. Surgery with a stable plate often allows earlier movement than casting.

Rehabilitation goals

Wrist bending and straightening, forearm rotation and grip strength are the main targets. Turning the palm up is commonly limited early on. We use heat or wax to ease stiffness before exercise, progress from active to assisted movement, and then introduce strengthening and functional tasks.

Tendon irritation from the plate occasionally occurs and is worth mentioning if you notice clicking or pain with thumb or finger movement. Most people recover good function within three to six months.
